Indigenous Community Participation in a TB Accompaniment Program

KantiHealth was honored to present at the 2024 Union World Conference on Lung Health, where our culturally tailored, community-based TB care model co-developed with Indigenous Community Health Workers of the Anak Dalam Tribe was highlighted as a promising public health practice in the Repertoire Session. Presented by co-founder Christy Abigail Wulandari, MD, the work underscored the overlooked TB burden among Indigenous peoples in Indonesia and showed how meaningful community participation and Indigenous leadership strengthen early TB detection, improve treatment initiation, and raise TB treatment completion rates to over 95 percent compared to below 35 percent in non-accompanied communities, reinforcing our belief that effective health solutions must be shaped with Indigenous communities to become more accessible, trusted, and effective.

CASE STUDY: Community-based Holistic Tuberculosis Management Initiatives

Community-based tuberculosis care led by Indigenous Community Health Workers (CHWs) of the Anak Dalam Tribe was featured as a case study in Health Equity: A Solution-Focused Approach, published by Springer Publishing. The chapter highlights how a culturally rooted, community-driven model strengthened early TB detection, improved treatment initiation, and increased treatment completion in this underserved Indigenous population. It also shows how oral storytelling, visual tools, and participatory learning enabled the community to engage actively in TB education and care, illustrating how Indigenous leadership and CHW-led approaches can overcome structural barriers and advance health equity in remote settings.

Characteristics of Desired and Feasible Community-based TB Screening Strategies

KantiHealth’s current research is part of our effort to learn together with the Anak Dalam community as we expand our work deeper into the forest, aiming to reach 1,000 community members by 2026. The study seeks to ensure that community-based TB care aligns with the community’s health needs and priorities while also matching the capacity and capability of the local health system to be strengthened. We use free listing, in-depth interviews, focus group discussions, and ethnography within the Health Equity Implementation Framework, working across sectors, starting with the Anak Dalam community itself, local healthcare facilities, local NGOs, district and provincial health and public affairs offices, as well as governmental partners. This work is supervised by the Department of Global Health and Social Medicine at Harvard Medical School and the Department of Community Medicine at the Faculty of Medicine, Universitas Indonesia.
